Social Work Care Manager – Direct Hire
Schedule: Monday–Friday, 9:00 AM–5:00 PM, alternating weekends and holidays
Hours: 75 hours per pay period
Pay: $37.45–$56.17 per hour
Job Summary
Complete comprehensive patient assessments, identify discharge-planning and readmission risks, coordinate care across hospital and post-acute settings, arrange aftercare services, educate patients and caregivers, and document all care-management activities.
Requirements
Master’s degree required
Active New York State LMSW or LCSW
Two years of acute-care hospital or related experience preferred
Electronic medical record experience
PRI/Screen certification within 90 days of hire
Strong care coordination, discharge planning, and interdisciplinary communication skills
