Salary: $95,000 a year

Social Worker Opportunity:

• Full-time, permanent position
  
• Schedule: Day shift; Monday-Friday

• Provide social worker services by delivering behavioral health interventions as part of an interdisciplinary care team to improve patients’ physical health and psychosocial outcomes
  
• Support individuals in managing chronic conditions through coaching sessions, medication and treatment plan adherence support, self-management education, goal setting, and facilitation of group classes
  

Social Worker Qualifications:

• Social Worker must have or be willing to obtain LCSW or LMFT in the state of Missouri
  
• Strong communication, organization, and multitasking skills with the ability to manage competing priorities and work effectively within a multidisciplinary team
  
• BLS required upon hire
