LCSW / LMSW Adolescent IOP- Norwalk Hospital
Summary:
Facilitates group therapy, completes psychiatric assessments, treatment planning, coordination of care, stabilization of patients and discharge planning. Strong collaboration with families, community providers/health care providers, and treatment team.
Responsibilities
1. Facilitates group therapy, provides individual, family, crisis intervention, patient-centered support and psycho-education.
2. Establishes objectives and develops patient-focused treatment plans to provide disposition and/or referral. Identifies potential barriers, limitations & lack of coping abilities, adjustments, and functional status. Participates in patient care planning.
3. Conducts intakes with patients/family members to evaluate coping abilities, dysfunctional adjustments, maladaptive behaviors, functional status.
4. Maintains knowledge of community resources, refers patients and families to agencies for supportive services with concrete resources (financial assistance, housing, transportation; medications) on regular & emergency basis.
5. Completes hospital and state required documentation, other paperwork/forms, and insurance authorization requirements.
6. Evaluates for child, elder abuse, and domestic violence and makes appropriate referrals. Acts as a consultant and/or educator to staff relative to assessment, criteria & referral.
7. Acts as a liaison for the patients within community (local, state and private agencies).
8. Fulfills all compliance responsibilities related to the position.
9. Coordinates weekly treatment team meetings.
10. Maintains census and will create community connections/relationships to generate referrals.
11. Performs other duties as assigned.
Education Skills Experience:
Master's degree in Social Work from an accredited school
Minimum of three years’ experience
Current Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (able to complete LCSW in 18 months)
Salary Range: $36.53 - $67.90 Hourly
