Job Summary and Qualifications

Part Time (Monday - Friday, days), no nights,noon-call, no weekends, no holidays 

 

Seeking a Social Worker Outpaient-Licensed for our practice to perform evaluations and services to patients of various age groups. Collects data and renders a professional opinion about treatment and care. We want you to apply now. 

 

Whatwill youdo in this role: 

 

Performs vocational rehabilitation assessmentin a timely manner; dictates interdisciplinary report to clinical record within the requiredtime frames. 

Initiates anddischarge documentswithinrequiredtimeframe. 

Documents oftherapynoteson the clinical record.  

Formulates and reviewsinitialand comprehensive treatment plans within the requiredtime frame. 

Identifiesand documents patient care needs.  

Works closely with the interdisciplinary team toassureappropriate patientcare. Knows, understands,maintainsand monitors patients’ rights. 

Provides patient/family emotional support andeducation  

Meets in-service requirements including mandatory education 

Perform other duties asrequired. Must read, understand, and adhere to all Physician Services policies and procedures. 

Practice andadherenceto the Code of Conduct and Mission and Value Statement.  

 

What Qualifications you will need: 

 

Education:  

Post Graduate degree 

 

EXPERIENCE: 

Minimum of 3-5 years’ experience  

 

CERTIFICATION/LICENSE:3 ways to qualify: 

A Clinical Mental Health Counselor license or Independent Clinical Social Worker license isrequiredin accordancewithstate regulations.
