Social Services Assistant/Designee - Licensed Social Worker Preferred 

Do you have a passion for advocating for older adults and supporting residents and families through important life transitions? Guardian Angels Health & Rehabilitation is seeking a compassionate, organized, and dependable Social Services Assistant/Designee - Licensed Social Worker preferred, to join our interdisciplinary care team.

In this role, you will help ensure that each resident’s emotional, social, and psychosocial needs are identified and addressed. You will work closely with residents, families, nursing staff, leadership, and community partners to coordinate admissions, care planning, advocacy, discharge planning, and other social services.

Key Responsibilities

  • Assist residents and their responsible parties throughout the admission process, including explaining facility policies, resident rights, advance directives, grievance procedures, contracts, services, and costs.

  • Advocate for residents and help address questions, concerns, complaints, and grievances.

  • Complete psychosocial assessments, Vulnerable Adult Status assessments, discharge planning goals, and other required documentation as delegated.

  • Coordinate interdisciplinary discharge planning to support safe and successful transitions.

  • Assist with admissions, room transfers, discharges, applicant tours, and communication with residents and families.

  • Maintain timely, accurate, and confidential social services documentation in accordance with facility policies and state and federal regulations.

  • Collaborate with residents, families, staff, healthcare providers, and community resources.

  • Participate in care center committees, staff meetings, training programs, emergency drills, and onboarding activities.

  • Follow all infection prevention, workplace safety, confidentiality, and emergency preparedness procedures.

  • Perform additional duties as needed to support residents and the ongoing operations of the care center.

Qualifications

  • Experience providing healthcare-related services is required.

  • A degree in social work, human services, or a related field is preferred.

  • Licensed Social Worker preferred.

  • Previous experience in long-term care, senior services, healthcare, or discharge planning is preferred.

  • Knowledge of social services practices and long-term care regulations is beneficial.

  • Strong communication, advocacy, organizational, and time-management skills.

  • Ability to manage multiple responsibilities while maintaining accuracy and confidentiality.

  • Ability to work independently and collaboratively as part of an interdisciplinary team.

  • Must demonstrate professionalism, compassion, patience, and respect when working with residents, families, visitors, and staff.

Physical and Workplace Requirements

This position includes standing and walking for approximately four to six hours per day and sitting or completing documentation for approximately two to three hours per day. The employee must be able to lift, carry, push, or pull at least 30 pounds and work throughout all areas of the care center.

The position may involve frequent interruptions, exposure to infectious materials, hazardous chemicals, odors, and other conditions commonly found in a healthcare environment. Emergency call-back may occasionally be required.
