Social Worker The Veterans Health Administration is seeking a full-time Social Worker to support adult and geriatric Veterans enrolled in the Home Based Primary Care (HBPC) Program at the Finger Lakes Healthcare System. This position provides case management, clinical treatment, advocacy, psychosocial assessment, treatment planning, and coordination with VA and community-based service providers. Location: Bath, NY Salary: $74,678 - $97,087 per year Pay Scale & Grade: GS-11 Work Schedule: Full-time, 7:30am - 4:00pm Appointment Type: Permanent Remote/Telework: Not available Travel Required: 75% or less Key Responsibilities: Provide direct social work services to adult and geriatric Veterans enrolled in the HBPC Program. Conduct psychosocial assessments in collaboration with Veterans, family members, caregivers, and interdisciplinary treatment teams. Contribute to treatment planning and establish achievable goals with Veterans and clinical staff. Serve as a subject matter expert on VA and community resources, referrals, and support services. Provide advocacy, crisis intervention, supportive counseling, and education for Veterans and caregivers. Coordinate non-institutional and institutional services, including HBPC, home hospice, adult day health care, and skilled or non-skilled home care services. Document Veteran and family contacts in the electronic medical record using appropriate formats and templates. Communicate effectively with community agencies, Medical Center staff, and interdisciplinary teams to support continuity of care. Basic Qualifications: Must be a U.S. Citizen. Must be proficient in written and spoken English. Must have a master's degree in social work from a CSWE-accredited school of social work. Must be licensed or certified by a state to independently practice social work at the master's degree level. GS-11 applicants must have at least one year of post-MSW experience equivalent to the GS-9 level in a health care or social work-related setting, or a doctoral degree in social work. Must pass a background/security investigation and pre-employment physical examination.
