Job Description

NATURE OF WORK

Under general supervision, provides social work and case management services to adult Town residents in need. This may include older adults, families, and adults with various disabilities, including physical, psychiatric, cognitive and substance-use disorders. 

ESSENTIAL JOB FUNCTIONS

  • Conducts a thorough intake to identify resident needs.
  • Screens residents for appropriate Town, State, Federal and private programs and services.
  • Assists with application processes when necessary and appropriate.
  • Make referrals and linkages to appropriate Town, State, Federal and private programs and services.
  • Consults and partners with other Town departments around issues of shared concern.
  • Consults and partners with State and Federal agencies, medical and mental health providers, local community-based organizations, and faith-based organizations regarding mutual clients and issues of shared concern.
  • Advocates on behalf of Town residents when necessary and appropriate.
  • Provides education and guidance around financial management, housing options and benefits/services.
  • Addresses hoarding cases and works with homeless residents and displaced residents, following established protocols where such protocols exist.
  • Follows all confidentiality protocols.
  • Documents all resident interactions in an electronic record system. 

 

OTHER JOB FUNCTIONS

  • Provides a variety of other social work duties as assigned or as is deemed necessary and beneficial. 

 

DESIRABLE KNOWLEDGE, SKILLS AND ABILITIES

  • Familiar and comfortable with case work methods and techniques.
  • Comfortable working with residents who display a range of personality types and behaviors, including residents with cognitive dysfunction, psychiatric illness, substance-use disorder, anger/outbursts and lack of impulse control. De-escalation skills are critical.
  • Comfortable working with residents with a range of physical conditions and abilities.
  • Proactive when networking and cultivating strong working relationships and partnerships with other Town departments and service providers.
  • Knowledgeable about cultural relativism, intersectionality, structural barriers and various forms of institutional oppression.

 

PHYSICAL AND MENTAL EFFORT/ENVIRONMENTAL AND WORKING CONDITIONS

  • Knowledge of program requirements and how to obtain community services.
  • Knowledge of legislation and regulations at the local, state and federal level which pertain to families, individuals, the elderly and disabled individuals and those declared incapable of handling their own affairs.
  • Considerable ability to communicate effectively, orally and in writing, and to establish and maintain effective working relationships with the general public, coworkers, community agencies and clients.
  • Ability to interview, counsel, and advise clients and/or their families.
  • Ability to make effective decisions under emergency conditions.
  • Ability to prepare written records and reports.
  • Ability to use or learn to use computers.

     

PHYSICAL AND MENTAL EFFORT/ENVIRONMENTAL AND WORKING CONDITIONS

  • Must be mobile and able to perform moderately difficult manipulative skills. Able to perform tasks which require hand-eye coordination. Able to sit for prolonged periods of time.
  • Must be able to see objects closely as in reading a report. Able to hear normal sounds with some background noise and able to communicate through human speech.
  • Able to concentrate on fine detail with constant interruption. Need to attend to a task/function for more than 60 minutes at a time. Able to understand and relate to specific ideas, generally several at a time and able to understand and relate to the theories behind several related concepts. Able to remember multiple tasks/assignments given to self and others over long periods of time.
  • Ability to get along with co-workers, supervisors, customers and the public at large.
  • Exposure to loud and unpleasant noises.
  • Exposure to seasonal conditions in outside weather.

     

MINIMUM TRAINING AND EXPERIENCE

Master’s in social work preferred or Bachelor's degree is required with courses in social work, gerontology, psychology, counseling or related social work subjects, and at least one (1) year's experience as a Social Worker performing direct services to individuals, the elderly or families or working with mentally and/or physically impaired clients in a public or private social services agency or medical facility. Must possess a valid Connecticut motor vehicle operator's license.
