About the Role:Join STARS Adult Medical Day Care Center as a Social Worker and make a meaningful impact in the lives of our clients. We are looking for compassionate professionals who are dedicated to providing support and guidance in a nurturing environment.
Responsibilities:- Conduct comprehensive assessments to determine client needs and develop individualized care plans.
- Provide counseling and support to clients and their families to enhance overall well-being.
- Coordinate with healthcare providers to ensure holistic care for clients.
- Facilitate group and individual therapy sessions as needed.
- Maintain accurate and confidential client records in compliance with regulations.
- Advocate for clients' rights and access to necessary resources and services.
- Participate in team meetings to discuss client progress and care strategies.
- Stay updated on community resources and services to better assist clients.
Requirements:- A degree in Social Work from an accredited program.
- Current state licensure as a Social Worker.
- Experience in a healthcare or social service setting preferred.
- Bilingual candidates are strongly encouraged to apply.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
