ASPIRUS ST. LUKE''S CLINIC in Duluth, MN is seeking a SOCIAL WORKER to join our CARE COORDINATION team!

Salary Range: $28.09 - $40.72 Hourly

Schedule/Hours: DAY ONLY, 8 hours

Weekend Requirement: Generally No Weekends

FTE: FULL TIME, 1.0 FTE

The Social Worker plays a vital role in delivering patient-centered care by assessing psychosocial needs, coordinating referrals, and addressing social drivers of health. As an integral member of the multidisciplinary care team, the Social Worker collaborates with physicians, nurses, and ancillary departments to support comprehensive care planning. This position focuses on resolving social, economic, and environmental barriers that impact a patient''s ability to manage their health effectively. Success in this role requires strong judgment, analytical thinking, problem-solving, and the ability to prioritize and manage multiple tasks in a dynamic healthcare environment.

Experience/Qualifications

Bachelor''s degree from an accredited school.

Knowledge of social work theories, therapies, and techniques of casework processes, and principles of public welfare. Previous social work practice in healthcare i.e. hospice, public health and human services. Inpatient preferred or inpatient hospital internship recommended.

Minimum of 1-2 years of experience in a healthcare or clinical setting preferred.

Knowledge of social work principles, ethics, and applicable laws and regulations.

Experience with care coordination, resource navigation, and patient advocacy.

Required

Possesses current Certified Social Worker licensure, within the state of responsible area or assigned business unit.

If current is temporary, obtainment of permanent license is required within 6 months of hire date.

As Aspirus operates across multiple states, Social Workers must obtain their Certified Social Worker licensure in Wisconsin, Michigan, and/or Minnesota within six months of hire based on their responsible area or assigned business unit to ensure compliance and support system-wide care delivery.

There is potential for on-call, weekend, and holiday coverage based on the responsible area or assigned business unit.

Collaborate with Providers, RN Care coordinators and Clinic Staff to ensure seamless communication of needs and outcomes. Provide warm handoffs as appropriate.

Develop relationships with community partners and agencies across the region.

Utilize independent judgement with ability to multi-task and manage a high volume of patients with complex needs.

Provide referrals to appropriate community resources.

Utilize critical thinking and creative strategies to address patients SDOH needs with often few options for resources due to funding and program limitations.

Ability to develop rapport and trust through various ways of communication with patients and families.

Manage complicated situations and emotions that patients and families are dealing with and assist with resolving issues.

Exhibits strong human relation skills for interacting with patients, patient''s family members, physicians, personnel, community agencies, and other resources.

Displays analytical skills in order to assess patients'' needs, develop associated discharge planning, and provide sound recommendations and guidance.

Displays ability to concentrate and pay close attention to detail when interviewing patients & preparing discharge plans.

Possesses planning and organizational skills and expert skills in prioritization.

Understands the nature of social diversity and oppression with respect to race, ethnicity, national origin, color, sex, sexual orientation, gender identity or expression, age, marital status, political belief, religion, immigration status, and mental or physical disability.
