Responsibilities Take charge of discharge coordination to ensure smooth, timely transitions of care that support both excellent patient outcomes and efficient patient flow within the hospital. Evaluate each patients clinical condition and psychosocial needs, identifying risk factors and designing a plan of care. Pinpoint essential interventions and work closely with physicians and primary nurses to tailor care plans to the patients specific situation and goals. Build trust and collaboration by working directly with patients and their loved ones, ensuring their voices are heard and their needs are met. Play a central role in care coordination rounds, complex care discussions, and unit-based team meetings, promoting clear communication and unified planning across the care team. QualificationsEducation Masters Degree in Social Work required. Licenses & Certifications Social Work license in Iowa and Illinois required. License in second state must be obtained within 30 days of hire. CPR certification upon hire. Valid drivers license when driving any vehicle for work-related reasons. Experience 1-2 years post-graduate experience. Specific offers are determined by various factors such as experience, skills, internal equity, and other business needs.
