Position Title: Social Worker Care Management
Location: Simi Valley, CA 93065
Duration: 5 weeks
Work Schedule: Day Shift (7:00 AM 7:00 PM, 5x8s)
Guaranteed Hours: 40 Hours/Week
Responsibilities for this position include, but are not limited to:- Assess patients' psychosocial needs and develop individualized care plans.
- Coordinate discharge planning and post-acute care services.
- Assist patients and families with transitions to hospice, rehabilitation, or community resources.
- Provide counseling, emotional support, and patient/family education.
- Collaborate with physicians, nurses, case managers, and outside agencies.
- Document assessments and care plans accurately in EPIC.
