Posting dates for internal transfers: July 9, 2026, through July 11, 2026 Job Code SW-LSW Job Description Title ACH-CM-SWKR Legal Entity Alliance Community Hospital Position Summary The Social Worker (LSW) provides social services for palliative and inpatient hospice patients and/or family as appropriate to meet the needs and treatment goals of the individual. The Social Worker (LSW) works collaboratively with the Palliative Care Team including the physician, nurse practitioner, chaplain, and other team members. Reports to Director of Case Management. Department Summary Case Management is comprised of RNs, LPN, and LSWs. The department assesses the discharge planning needs of the adult patient while hospitalized. Case Management orchestrates referrals to outside agencies and facilities to best meet the discharge needs of the patient in a safe and timely manner. Palliative care focuses on patients who need specialized care for serious illnesses. The focus of palliative care is to provide relief of symptoms of said illnesses. The goal is to promote improved quality of life. The Alliance Community Care Network, (ACCN), focuses on chronic disease management and addressing the socioeconomic, educational, psychosocial, and behavioral factors, as well as each persons adherence potential with the goal to form seamless delivery of services for the targeted populations and maximize the impact of resources. The ACCN-MSW is a licensed MSW or LISW who works to coordinate the psychosocial economical needs of Alliance Community Hospital patients with a focus on quality-of-care initiatives and improved patient outcomes. Qualifications Licensed Social Worker with current Ohio License. Bachelors or Masters degree in Social Work or related degree from accredited school with eligibility for licensure in state of Ohio. One-year experience in social work at an acute care facility is preferred or previous work experience in a social agency or social service department of a health agency. Department Qualifications Skills Acts as a patient/family advocate. Must be able to work autonomously within a team concept. Able to manage time effectively Must be computer literate to complete task(s) as required. Such as Cerner (Hospital Support Software), Halogen (performance appraisal system) Relias (education system), ability to retrieve e-mails, complete online enrollment for health insurance/benefits, and any other programs as assigned. Department Skills Responsibilities Expectations Provides comprehensive assessments of patient/family needs, as these relate to the episodes of acute and/or chronic illness or crisis, including transitions into Palliative care or hospice care. Works to establish treatment goals of care and provide alternatives to assist the patient and/or family in the hospital, palliative care clinic, oncology office, or ACCN. Collaborate with the RN CM or IP LSW for coordination of discharge planning. Collects quality data as instructed by the quality department or director of case management and participates in ACCN and Palliative care outcome analysis. Acts as a liaison between hospital and available community resources and participates in conferences, agency meetings, etc. to increase professional knowledge. Participates in hospital and palliative care committees as appropriate or directed by the director of case management. Participates in daily multidisciplinary rounds. Communicate effectively with patients and families. An emphasis is placed on compassion and empathy. Ensures assessment includes the patients social, financial, functional, environmental, physical, mental, and spiritual factors and assist in developing the Palliative/AACN plan of care. Documents timely in the medical record. Counsels and provides therapeutic interventions to the patient and caregiver to support care decisions related to discharge disposition and/or living arrangements. Provide education and social support for patients and families that are participating in the ACCN or outpatient Palliative Care clinic as well as the oncology practice. Bereavement care in accordance with Bereavement plan of care. Manages time appropriately and independently between ACCN home visits, Inpatients, and outpatient Palliative Care clinic. Calls weekly or monthly, patient preference, on ACCN patients to inquire about any needs or concerns. Perform SCU psychosocial assessment when needed to assist coverage in SCU or when on call. Assist during the day shift suicide assessment for those who respond to OP questioning of suicidal ideation. Working Conditions Works 37.5 hours per week Call on weekends and holidays assigned, every 3rd weekend and holiday. On call rotations include the inpatient portion of hospital. Cross training for the other SW positions in hospital as coverage of those job duties may be required. Reports to the Director of Case Management. The normal work routine involves no exposure to blood; body fluids or tissues (although situations can be imagined or hypothesized under which anyone anywhere might encounter potential exposure to body fluids.)
