Social Worker Case Management

How you'll make an impact in this role:

• Design personalized care journeys across the full continuum of care, integrating patient and family preferences to ensure compassionate, values-based outcomes.

• Serve as a trusted expert in crisis and psychosocial support, evaluating complex needs—from substance abuse to legal or adjustment challenges—and creating tailored treatment recommendations.

• Fiercely advocate for patients by identifying systemic barriers and securing critical healthcare and community resources to enable smooth care transitions.

• Drive cross-functional team alignment through continuous dialogue with supervisors and care transition teams, ensuring health plans are proactively evaluated and adapted.

• Guide post-discharge success and documentation excellence by advising care teams on accurate medical records, complex levels of care, and diagnostic-driven treatment options.

What minimum qualifications you'll need:

Licensure / Certification / Registration:

• One or more of the following required:
  • Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
