How you'll make an impact in this role

• Complete timely psychosocial assessments, document care plans, and support residents'' emotional and social well-being.
• Collaborate with the interdisciplinary team, residents, families, and community partners to ensure quality care and smooth care transitions.
• Assist residents and families with community services, financial assistance, discharge planning, and other support resources.
• Identify safety concerns, respond to changing resident needs, and advocate for resident rights while maintaining compliance with healthcare regulations.

What minimum requirements you'll need

Licensure / Certification / Registration:

• Master Social Worker, preferred.

Education:

• High school diploma/GED with 2 years of experience, or Associate''s degree, or Bachelor''s degree required.

Work Experience:

• 3 years of experience preferred.

What additional preferences we''re seeking

• Long term care experience highly preferred.
• Clinical Social Worker Licensed highly preferred.
