    Role Description This is a full-time, on-site Social Worker role based at Fulton Commons Care Center in East Meadow, NY. The Social Worker will provide psychosocial support to residents and their families, including conducting assessments, developing care plans, and facilitating transitions into and out of the facility. Daily responsibilities include leading or participating in care plan meetings, coordinating discharge planning, providing counseling and crisis intervention, and advocating for residents’ rights and preferences. The Social Worker will collaborate closely with nursing, therapy, and administrative teams to address social, emotional, and environmental needs and to help residents access community resources and services. The role also involves maintaining accurate documentation, complying with regulatory requirements, and participating in quality improvement initiatives.

    • Demonstrated skills in psychosocial assessment, care planning, and case management in a healthcare or long-term care setting.

    • Ability to provide counseling, crisis intervention, and supportive communication to residents, families, and caregivers.
    • \n
    • Strong collaboration and advocacy skills for working with interdisciplinary teams and community agencies.
    • \n
    • Proficiency in documentation, record-keeping, and adherence to regulatory and ethical standards.
    • \n
    • Bachelor’s or Master’s degree in Social Work or a related field; relevant licensure or eligibility for licensure as required by New York State.
    • \n
    • Knowledge of long-term care, rehabilitation, and community resources, with experience in discharge planning preferred.
    • \n
    • Strong organizational skills, reliability in a full-time on-site environment, and the ability to manage multiple cases effectively.
    • \n
