Working for this leading academic medical center means collaboration with top clinical, technical and business professionals across all disciplines.The Behavioral Health Social Worker is a master's level behavioral health professional who conducts clinical assessments and provides brief interventions, care management, navigation and proactive follow up for patients with comorbid medical, mental health and/or substance use disorders. The Behavioral Health Social Worker leverages their behavioral health expertise in providing care to patients and families, improving transitions of care and assuring that appropriate follow up care is established to assure the best possible clinical outcomes for at risk patients. The Behavioral Health Social Worker is also competent in nonbehavioral health related resources and services (medical home care, office of aging, children and youth, domestic violence, etc) to facilitate discharge planning in the Emergency Department.
Master of Social Work required.
License of Social Work(LSW) preferred.
