SOCIAL WORKER FOR PERLMAN CENTER
Schedule: This position is full-time, Monday-Friday 8AM-4:30PM., 40 hours/week.
Location: Burnet Campus with occasional visits to satellite offices.
Required Job Qualifications:
The Perlman Center is a specialty center serving patients with cerebral palsy and similar neuromotor conditions from birth through adulthood. The Perlman Center provides evidence-based PT, OT and Speech therapies in a group setting with a multi-disciplinary team (early intervention, pre-school, school-age, and young adult small groups) and provides a wide range of specialty assistive technology evaluations and treatment.
The Perlman Center currently has a team of six social workers who are based at the Mason and Main Hospital locations and provide psychosocial assessments, caregiver support, care coordination, and resource navigation for patients and families served at the Perlman Center and in Pediatric Rehab clinic visits at multiple satellite locations.
We are now recruiting a Social Worker to join the Perlman Center team. As an active member of the Perlman multi-disciplinary care team, you will complete psychosocial assessments, provide clinic support for interdisciplinary CP Clinics in Pediatric Rehabilitation and Medicine (PMR), and participate in team planning and program administration to ensure optimal care for patients and families. Perlman Center social workers also facilitate in-person and virtual programs providing education and mental health support for parents and siblings of children with CP.
