Job Title: Medical Social Worker (Hospice)

Location: Forrest City, AR (Travel to Patient Homes Required)
Office Location: 4200 N. Washington, Forrest City, AR
Schedule: Monday–Friday | 8:00 AM – 4:30 PM (Excluding Holidays)
Position Type: Full-Time
Pay: $30/hr (Paid Weekly)

Position Summary

We are seeking a compassionate and dedicated Medical Social Worker to join a collaborative hospice care team. In this role, you will provide psychosocial support and counseling to hospice patients and their families, helping them navigate the emotional, social, and practical challenges associated with serious illness. Working as part of an interdisciplinary care team, you will develop care plans, connect patients with community resources, and promote continuity of care while ensuring patients and families receive the highest level of support.

This position requires travel to patient homes throughout the service area.

The team is committed to delivering compassionate, patient-centered hospice care through collaboration and ongoing support.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments to evaluate patients' and families' emotional, social, and environmental needs
  • Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary hospice team
  • Provide counseling, education, and emotional support to patients, caregivers, and family members
  • Assist patients and families in understanding medical recommendations and available treatment options
  • Connect patients with appropriate community resources and advocate for needed services
  • Identify gaps in community resources and support resource development initiatives
  • Complete home visits according to the established Plan of Care and submit accurate clinical documentation daily
  • Collaborate with hospitals, healthcare providers, and community agencies to promote continuity of care
  • Participate in interdisciplinary team meetings and contribute to patient care planning
  • Educate hospice staff on psychosocial issues and available community resources
  • Facilitate monthly caregiver support groups
  • Serve as a mentor and preceptor for newly hired social workers as assigned
  • Assist with the development and revision of hospice social work and bereavement policies
  • Provide quarterly supervision and education for assigned social workers
  • Serve as Bereavement Coordinator or provide support in bereavement services as needed
  • Complete all required training modules and attend mandatory in-service education

Required Qualifications

  • Master's Degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE)
  • Minimum of one (1) year of social work experience in a healthcare setting
  • Current CPR Certification
  • Current driver's license, valid auto insurance, and reliable transportation
  • Ability to travel to patient homes throughout the assigned service area
  • Excellent communication, counseling, and case management skills
  • Strong organizational and documentation abilities

Licensure Requirements

  • Current Social Worker license issued by the applicable State Board of Social Work (required in Arkansas and other applicable states)
  • Candidates must meet all state-specific licensing requirements for the state in which they practice

Preferred Qualifications

  • Previous hospice, home health, palliative care, or case management experience
  • Experience working within an interdisciplinary healthcare team
  • Knowledge of community resources and end-of-life care services
  • Leadership or mentoring experience
