This role is vital in supporting a small, focused caseload of adolescents (6th–12th grade) who require intensive therapeutic intervention within an educational setting. As a key member of a multidisciplinary team, you will bridge the gap between mental health treatment, family dynamics, and school re-entry.
Key Responsibilities

  • Clinical Caseload Management: Maintain a dedicated caseload of approximately 8 children, providing high-touch clinical support and advocacy.

  • Therapeutic Services: Deliver comprehensive care including Weekly Individual Therapy, Daily Group Therapy, and Family Therapy as needed.

  • Admissions & Screening: Collaborate with families, medical doctors, and school liaisons to participate in the IDT admissions process and facilitate referrals across Orange County.

  • Interdisciplinary Collaboration: Function as a core member of the treatment team, participating in weekly meetings to ensure integrated care.

  • Transition & Discharge Planning: Lead transition meetings involving families, teachers, and home-district professionals. Ensure thorough discharge planning and consistent follow-up post-treatment.

  • Crisis Intervention: Provide immediate support and de-escalation for youth and families during clinical crises.

  • Advocacy & Community Linkage: Act as a bridge to community-based services, ensuring families have the necessary support systems beyond the IDT program.

  • Documentation: Maintain precise and timely medical records in compliance with regulatory standards.

Qualifications

  • Education: Master’s Degree in Social Work (MSW), Mental Health Counseling (MHC), or a related clinical field.

  • Licensure: Active New York State clinical license (LMSW, LCSW, LMHC) preferred.

  • Experience: Proven experience working with adolescents (middle and high school age) in a therapeutic or school-based setting.

  • Skills: * Strong group facilitation skills.

    • Expertise in crisis intervention and de-escalation.

    • Ability to collaborate effectively with school districts and external agencies.

    • Exceptional organizational skills for managing medical documentation and discharge timelines.
Why Join This Team?
This is a high-impact role where you can see the direct results of your work as students transition back to their home districts with improved coping skills and stability. You will work in a supportive, interdisciplinary environment that values clinical expertise and family-centered care.
