Shift: Day | 8:00 AM–4:30 PM
Schedule: Monday–Friday | 5×8 hours | 40 hours/week
Local Candidates: Not accepted within 50 miles
Traveler Candidates: Accepted
Pay Rate: $45–$50/hr

Requirements:

• 2+ years recent Social Worker experience
• Active Georgia LCSW license
• AHA BLS
• Acute hospital experience required
• Hospital-based social work experience
• Strong assessment and care-coordination skills

Responsibilities:

• Conduct psychosocial assessments
• Provide emotional support and counseling
• Assist with discharge planning
• Coordinate referrals and community services
• Identify discharge barriers
• Collaborate with physicians, nurses and case managers
• Maintain accurate documentation
• Support continuity of care

Holiday Requirement: 2 major holidays + 1 minor holiday.
