Location: UF Health Flagler Hospital

400 Health Park Blvd St. Augustine, Florida 32086  

Schedule: Monday - Friday 8:00am - 4:30pm | 40hrs. per week

The Medical Social Worker is responsible for the provision, direction, supervision and evaluation of medical social 
services to clients in the home environment, in accordance with agency policies and procedures, applicable health 
standards, governmental laws and regulations. Proactively meet the needs of and assist the client in the achievement of 
individualized and measurable outcomes.

Responsibilities:

• Instruct and counsel the client and family in treating and coping with the social and emotional responses to illness.
• Instruct, treat, observe and evaluate clients with significant social and emotional situations affecting their health status.
• Participate in care coordination activities and act as resource to other team members in identification and resolution of client needs.
• Participate in development and re-evaluation of Plan of Care for clients needing MSW.
• Participate in discharge planning.
• Provide ongoing assessment of needs.
• Report any changes in client's condition and response to treatment to supervisor and the physician.
• Review LSW documentation for accuracy with plan of care and cosigns.
• Supervise Bachelor’s level social workers.
• Perform all other duties as assigned by management within job scope.

Qualifications:

Education

• Bachelor's Degree Social Work or Psychology or related area of study.

Experience

• 1 year Medical Social Work experience.

Certificates/Licenses/Registration

• Level 2 Background
• MVR Background Check - Must have a good driving record. Valid driver's license and proof of auto insurance required for Motor Vehicles Record (MVR) background check
• Alzheimer’s Disease and Related Disorder (ADRD) 2-hour course (Home Care Only) required within 90 days of hire for Home Care only.
