Description
Directly provides counseling, case management and other chemical dependency services in meeting the social emotional needs of the detox/rehab patients and their family members.
Responsibilities:
Provide assigned patients with activities, group sessions to plan objectives, stimulate behavioral change and to process effectively.
Provide evaluations and psychosocial assessments for patients in collaboration with the td team.
Initiate, develop, coordinate and evaluate treatment plans in providing appropriate treatment for individual patients.
Prepares and keeps a current case record for each assigned patient according to HealthAllianceHospital, OASAS, and JC standards.
Participate as a team member in conferences, treatment planning on behalf of patient.
Attend in-service training and keeps professionally updated through reading and attending seminars.
Provide lectures, workshop/training for patients and staff as assigned.
Strictly adheres to all of the confidentiality requirements imposed by law and professional conduct.
Observes adherence to current policies and a resource to director for program recommendations.
Perform any other duties as assigned.
Qualifications/Requirements:
Experience:
Minimum one (1) year experience providing mental health treatment preferred.
Experience using one or more evidence-based treatment approaches preferred.
Experience working with individuals with SUD preferred.
Education:
Masters of Social Work or Mental Health Counseling from an accredited program.
Licenses / Certifications:
Current NYS Masters Level licensed QHP or licensure eligible with limited permit.
