    Schedule: Monday-Friday Day shift

    This position primarily serves participants in PACE''s East Region, including the Gresham and Glendoveer service areas. Services are provided across clinic, home, residential, and community settings.

    The Mental Health Social Worker (LCSW) is a member of the interdisciplinary team (IDT) and provides behavioral health assessment, treatment, consultation, and care coordination for older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE). This position supports participants experiencing mental health concerns, emotional distress, adjustment challenges, serious mental illness, substance use concerns, cognitive impairment, and complex psychosocial needs.

    Working collaboratively with participants, caregivers, community partners, and the IDT, the Mental Health Social Worker helps promote emotional well-being, behavioral health stability, independence, and quality of life while supporting participants to remain safely in the community whenever possible.

    Required Qualifications:

    • Master''s degree in Social Work (MSW) from an accredited school of social work.
    • Current Oregon Licensed Clinical Social Worker (LCSW) license.
    • Minimum of two years of post-licensure clinical experience in mental health practice.
    • Experience working with older adults or individuals with complex medical, behavioral health, or psychosocial needs.
    • Strong assessment, counseling, crisis intervention, and care coordination skills.

    Preferred Qualifications:

    • Experience in PACE, geriatrics, integrated care, primary care, or community-based healthcare settings.
    • Experience working with dementia, serious mental illness, substance use disorders, and co-occurring conditions.
    • Training in evidence-based behavioral health interventions.

    Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate''s placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
