Position Summary

The Per Diem Licensed Clinical Social Worker (LCSW) provides comprehensive social work services to patients and their families, helping them navigate emotional, psychosocial, financial, and discharge-related needs. The LCSW collaborates with the interdisciplinary care team to assess patient needs, provide appropriate support and counseling, and connect patients and families with community resources.

Key Responsibilities

  • Provide psychosocial assessments, counseling, and emotional support to patients and families.
  • Assist patients and families with discharge and transitional planning, including coordination of appropriate post-acute services and community resources.
  • Educate patients and families regarding available financial assistance programs, benefits, entitlements, and community resources.
  • Identify and address psychosocial barriers that may impact a patient’s treatment, recovery, or discharge plan.
  • Provide crisis intervention and utilize appropriate de-escalation techniques when needed.
  • Conduct suicide risk assessments when indicated and communicate findings to the appropriate members of the care team.
  • Identify, assess, document, and report suspected abuse, neglect, and domestic violence to the appropriate protective service agencies in accordance with applicable requirements.
  • Collaborate with physicians, nurses, case management, and other members of the interdisciplinary team to support quality patient care.
  • Maintain appropriate documentation and ensure timely communication regarding patient and family needs.

Required Qualifications

Education

  • Master’s Degree in Social Work (MSW) from an accredited school of social work.
  • LCSW required.

Licensure

  • Current New York State Licensed Master Social Worker (LMSW) license.

Experience

  • 1–3 years of relevant social work experience required.
  • Experience in a healthcare setting preferred.
  • Experience in specialty areas such as dialysis, rehabilitation, substance use/ETOH, home care, oncology, or hospice preferred.
  • Experience with discharge and transitional planning.
  • Knowledge of community resources, entitlement programs, and eligibility requirements.
  • Strong counseling, crisis intervention, and de-escalation skills.
  • Experience providing psychological and emotional support to patients and families.
  • Knowledge and experience identifying, assessing, and reporting suspected abuse cases.

Additional Requirements

  • SBIRT certification required or must be obtained as required by the organization.
  • NYS DOH Screen Certification required or must be obtained as required by the organization.
Posted Salary Range: USD $53.09 - USD $53.09 /Hr. :

The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits
