Social Worker, LCSW Play Therapist (4284)

Position Summary:

The Social Worker, LCSW provides supportive, goal-directed social work services to patients, families, and support networks within the behavioral health and hospital setting. This role conducts comprehensive psychosocial assessments, participates in interdisciplinary care planning, and assists patients in accessing resources that promote safety, stabilization, and overall well-being. Working collaboratively with nursing staff, providers, and interdisciplinary team members, the social worker supports discharge planning, crisis intervention, and coordination of community-based services. Through compassionate communication, clinical insight, and patient advocacy, this position helps individuals understand treatment recommendations and navigate available supports throughout their care experience.

Key Responsibilities:

• Perform psychosocial assessments to identify patient needs, strengths, barriers, and available support systems.
• Participate in the development and implementation of care plans, collaborating with treatment teams to support patient stabilization and recovery.
• Assist patients and families with discharge planning, service referrals, and connection to community resources and support agencies.
• Provide crisis intervention, supportive counseling, and patient education within the scope of practice for a Licensed Baccalaureate Social Worker.
• Document assessments, care coordination activities, and patient interactions accurately and timely in the electronic health record.
• Maintain effective communication with interdisciplinary staff, external agencies, and family members to promote continuity of care and safe transitions.

Licenses and Certifications Required:

• LCSW in North Dakota.

Educational Requirements:

• Master's degree or above.

Experience Requirements:

• Requirements for LCSW licensure are completed.

Special Skills or Training Requirements:

• As determined by the Department Manager or needs of clinic.

Physical Requirements:

The Social Worker, LBSW frequently walks and moves throughout patient care units to meet with patients, families, and staff. The role requires periods of sitting for documentation, conducting interviews, and completing care coordination tasks. Adequate vision, hearing, and communication abilities are essential for engaging with patients, reviewing documentation, and participating in team discussions. Occasional light lifting of materials or patient belongings may be required.

Environmental Requirements:

This position functions within inpatient behavioral health and acute care settings, involving regular interaction with individuals experiencing emotional distress, behavioral changes, or complex psychosocial needs. Exposure to typical clinical risks-including infectious agents, cleaning agents, and moderate noise levels-may occur. The role requires adherence to safety protocols, de-escalation practices, infection-prevention procedures, and strict confidentiality standards to ensure a safe, therapeutic environment.
