Responsibilities


*Please note, this position is a non-benefitted, on-call position. The role will be hybrid, with possible remote option, after initial onsite training.


This position provides the clinical supervision of MSW students and ASW interns, as required by the Board of Behavioral Sciences. The hours will likely be around 8-16 hours per week, as needed, depending upon student schedules (reduced hours during school closures during the year and summer). Wednesday-Friday availibility preferred (to cover days when students are in offices).


Position Summary:


The Social Worker functions as a leader of the Dignity Health care interdisciplinary team (IDT) collaborating with all departments to ensure that a comprehensive treatment plan of care is developed monitored and amended as necessary and that appropriate outcome measures are identified that demonstrate attainment of individual patient goals.


The Social Worker will assess plan implement coordinate monitor and evaluate options and services to meet a patients need within an assigned case load using available resources to assist in achieving an effective treatment plan while promoting quality health care with cost effective outcomes. Special attention to detail in the patient populations psychosocial needs and assistance is a must.


The Social Worker works closely with the RN Care Coordinator to coordinate care plans based on the utilization review activities performed by the RN Care Coordinator with compliance of federal state and third party payers.


The Social Worker will be required to participate in performance improvement initiatives related to appropriate patient care management and perform other departmental duties as assigned. The care team program improves the quality of care and clinical outcomes for members with complex care needs by coordinating care within the health care delivery system while collaborating with a multi-disciplinary team. This team provides case management services to adults within Mercy Medical Groups continuity of care context including advocacy for services within the community and social services systems.


Minimal travel required (less than 25%).


Qualifications


Minimum Qualifications:


- Two (2) years clinical experience in a clinical setting performing clinical social work or equivalent combination of education and experience required.

- Masters Degree from a School of Social Work accredited by the Council of Social Work Education

- California Licensed Clinical Social Worker (LCSW)


Preferred Qualifications:


- Three (3) years of experience post licensure with an emphasis on mental health and case management experience preferred.

- Experience working with geriatric population and substance abuse patients preferred.
