Social Worker - Licensed

Duties include Completing admission paperwork, care plan development, completing resident assessments, overseeing resident discharge process, addressing resident and family concerns, leading resident care conferences.  Candidates must be able to work independently but will collaborate with and report to the Social Services Director.  Full-time, flexible day schedule between the hours of 8:00am - 5:00pm (hours per shift is also flexible). Range- $27.93 - $34.23. This is a new position that FHS plans to fill in July/August 2026.
