The Role The Social Worker provides each resident's social, emotional and psychological needs and supports the continuing development of the resident's full potential from admission to discharge. Responsibilities: Provide comprehensive social services support to residents by: - Developing a social history, social assessment and care plan identifying pertinent problems and needs, realistic goals and the specific action to be taken for resolution upon admission of each new resident.
- Participate in the pre-admission screening of all potential residents and provide information to residents and families during admission.
- Orient the resident to assisted living care environment and facilitate adjustment upon placement.
- Document progress notes that relate to each resident's care plan when necessary.
- Assist the resident and family in discharge and placement planning.
- Participate in interdisciplinary meetings, care plan meetings and quality improvement initiatives.
- Establishing working relationships with community health, welfare, and volunteer groups to promote cooperative assistance in meeting the social welfare needs of residents and families.
- Supporting residents in maintaining their highest level of independence and well-being.
- Documenting interactions and interventions in compliance with regulatory and facility guidelines.
- Assisting with arrangements for out-of-facility healthcare, transportation, and counseling services as needed
- Adhere to and model the way in all leadership competencies at Dow Rummel.
Who You Are - High school diploma or equivalent.
- Minimum of three years of experience working with older adults, with at least one year in a long-term care facility.
- College coursework in sociology, psychology, gerontology, or social work may substitute for up to two years of experience.
- Familiarity with residents' rights, long-term care regulations, and facility policies and procedures.
- Strong communication and interpersonal skills to effectively work with residents, families, and interdisciplinary teams.
- Ability to maintain confidentiality and comply with HIPAA regulations.
