JOB OPENING:          Social Worker – Long-Term Care in Skilled Nursing Facility - Full Time

 

LOCATION:                Northeast Minneapolis, MN

 

Catholic Eldercare is seeking an organized, person-centered Social Worker to join our team in coordinating services and meeting the psycho-social needs of our residents and their representatives.

 

Catholic Eldercare offers compassionate, person-centered care, housing, and services to all seniors across the continuum of care. For over 40 years, Catholic Eldercare has been a vital part of the NE Minneapolis community meeting the needs of elders in a caring, dignified atmosphere. We are seeking staff who share these goals to join our team!

Responsibilities:

The Social Worker will:
• Complete admission and discharge paperwork for residents.
• Monitor the psycho-social and emotional needs of residents.
• Create care plans and make amendments as needed.
• Coordinate all parties in resident care conferences/special meetings with a multi-disciplinary team.
• Complete assigned MDS interviews and MDS social services assessments.
• Assist residents and families with room changes.
• Oversee Medicaid- and Medicare-related paperwork and recordkeeping.
• Respond to resident/family/staff concerns using confidentiality and professional judgment.

Qualifications:

Required Qualifications:
Degree in Social Work or related
Current MN Licensure in Social Work or ability to successfully obtain license within 60 days.
Proficiency in Microsoft Office and digital literacy for charting and interfaces with outside services.
Ability to pass NetStudy 2.0 background screening.

Preferred Qualifications:
Experience working with individuals experiencing dementia and cognitive decline.

Job Context: Salary $57,000-66,000 per year, exempt (not eligible for overtime)
In-person position, on-site, 100% time.
