Social Worker - Long-Term Care

Blakeford at Green Hills, a non-profit continuing care retirement community nestled on ten acres in beautiful Green Hills, is actively seeking an experienced Social Worker to guide the resident experience in a long-term care environment.

As a Blakeford Social Worker, you will:

• Counsel residents and their families on emotional, environmental or physical observations and interventions.
• Conduct regular face-to-face visits with assigned residents 
• Facilitate active participation in care planning 
• Serve as resident liaison for social, health, and community services that meet individual needs throughout discharge planning activities.
• Compile performance metrics and present summary data
