Job Summary

Works with the Director and staff to develop, implement and sustain Social Work processes/practices that enhance the hospitals vision. Continuously analyze current strategies and research opportunities to improve processes around discharge planning and staff development to maximize the capabilities of department and to decrease LOS while maintaining patient satisfaction. Collaborate with and optimize relationship/communication with community agencies to develop partnership and facilitate a continuum of care.

Responsibilities

  • Manage the Social Work department activities by participating in the planning and development of goals and objectives, establish and implement policy and procedures for DNV surveys to ensure the Social Work program meets regulatory standards, laws, and Medical Center Mission and goals. Maintains knowledge of DNV standards and their interpretations as related to areas of responsibility.
  • Supervise the Social Workers on day to day operations.
  • Schedule and provide for effective coverage of all areas designated as requiring Social Worker coverage.
  • Develop, implement, and oversee standards for Social Workers. Evaluate Social Worker compliance with standards.
  • Conduct interviews and hire new staff based on ability to perform to the standards set by CAMC.
  • Provide/ participate in the training and orientation of new Social Workers in area of clinical social work, discharge planning and patient placements.
  • Prepare and conduct performance appraisals of Social Workers and makes recommendation for pay increases.
  • Arrange and oversee MSW field placement program for CAMC.
  • Assist in supervision and evaluation of personnel and other department personnel as necessary.
  • Interpret and administer hospital personnel policies as the affect personnel.
  • Serve as a resource and provide expert advice to Social Workers and other staff regarding social needs, placements and community agencies/resources.
  • Participate/conduct weekly team meetings with Social Workers/Case Manager as needed/required.
  • Assist with and assure completion of discharge plans and referrals for patients who are high risk.
  • Oversee the day to day management/supervision of the discharge planning process with unit focus on decreasing LOS.
  • Coordinate with nursing department staff and managers to ensure combined commitment and understanding to effective d/c planning process.
  • Manage contracting with outside agencies.
  • Make rounds to educate and ensure case management staff are following processes that adhere to standard/ department policy.
  • Maintain department, institution, and licensure (if applicable) required education. Document appropriately.
  • Responsible for updating and communicating changes to Social Workers/ Case Managers in a positive, timely and appropriate manner.
  • Responsible for integrating continuous, quality improvement into department and institution operations, and demonstrate a commitment to the process.
  • Reinforce employee understanding of work integration with other departments throughout the institution.
  • Exhibit professional behavior and serve as an effective role model through appearance, attendance, commitment, and competence.
  • Assist staff to work as a team toward the achievement of departmental and CAMC goals.
  • Encourage professional growth of staff.
  • Update/ revise training materials as needed.
  • Keep and maintain resource/referral list.
  • Serve on committees as social work representative as necessary.
  • Serve as community contact for social work services.
  • Collaborate with community agencies to facilitate and maintain good working relationships and enhance services to the individuals we serve.
  • Provide in-services to departments and community as requested on topics to social work and health care.
  • Responsible for quality improvement activities and continuing education.
  • Assist with developing/monitoring departmental quality improvement indicators.
  • Prepare and submit productivity reports and records concerning departmental activities.
  • Provide recommendation to Corporate Director on staffing, resource utilization and operations.
  • Communicate to staff, co-workers, and supervisor in a positive, timely, and appropriate manner.
  • Investigate and assist in resolution of problems involving patients, physicians, departmental staff and discharge plans.
  • Develop, revise and maintain policies as assigned.
  • Introduce and explain/instruct staff on new/revised policies/ equipment and processes.
  • Collaborate with Clinical Date Analyst to coordinate scheduling of software/ hardware upgrades, routine maintenance, and emergency repair sessions. Report equipment failures to appropriate personnel.
  • Participate in planning and implementing change and maintaining and improving productivity through attendance and participation in staff meeting, committees, task forces, projects and discussions as assigned.
  • Demonstrate very good interpersonal skills by using sound judgement and tact at all times.
  • Communicate and keep the RCCM Director appraised of patterns.trends associated with discharge plans (i.e., physicians, patient populations, units, etc.)
  • Assist with planning and implementation of short and long term projects to improve the outcomes of the case coordination program and continuous quality improvement activities of the institution.

The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department's identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients.

Education

  • Master's Degree (Required) Experience: 7 - 10 Years - 7 Years Total Related Experience With 3 Years of Which Must be in Acute Care Social Work
