Social Worker- Hospital Setting
Inpatient Units- 13 weeks, M-F, 8:30-5p
Required:
MD LMSW or LCSW-C
Must have recent hospital discharge planning experience in Med/Surg or ED
EPIC experience.
Description: Early identification, high-risk screening, and readmission screening of all patients. Services include, but are not limited to, adjustment to illness counseling, safety assessments, discharge planning, and identification of obstacles and barriers to discharge.
