    Job Description

    Social Worker MSW at Providence Santa Rosa Memorial Hospital in Santa Rosa, CA. This position is Full-time and will work 8-hour, Day shifts.

    The Social Worker provides comprehensive services to include psycho-social assessments, community resource information and referrals to patients, their families and loved ones, ranging in age from infants through geriatrics, as it supports the continuum of care. Provides coordination of complex discharge planning needs and facilitates multidisciplinary care team involvement to expedite plan of care for patients.

    Required Qualifications:

    • Master''s Degree: Master's degree in social work from an accredited CSWE program.
    • 1 year of Social Work experience in any healthcare setting.
    • 1 year of experience in an acute care setting or successful completion of TIPs program or Case Management Orientation Program.

    Preferred Qualifications:

    • Upon hire: Clinical Social Worker License (LCSW), or license eligible
    • Basic understanding of acute medical surgical healthcare terminology and prior case management experience.

    Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate''s placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
