Traveling to and working within both Morris County & Part of Passaic County. Please note that travel to these counties is a required part of the position (travel to homes, nursing homes, hospitals, assisted living facilities etc.)
Principal Accountabilities:
Performs comprehensive psychosocial assessments. Identifies behaviors and issues that affect coping at the end of life.
Identifies issues that may have an impact on the safety of patients and families Identifies availability of support systems to meet patient/family goals.
Identifies bereavement risks and communicates these appropriately. Collaborates with other disciplines in the development of the plan of care.
Counsel's patients and families in strategies to cope with issues of loss and terminal illness.
Participates with other disciplines in family conferences.
Provides referrals to appropriate community services.
Knowledge of age-appropriate behaviors, needs and interventions.
The ability to work with people of diverse cultures and socioeconomic status.
Facilitates movement between levels of hospice care.
Participates in the agency’s Performance Improvement program.
Assists with growing agency business. Cross trains to learn new skills
