The responsibilities of the Social Worker - MSW include, but are not limited to: 

Responsibilities:
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must have advanced training in hospital or long-term care administration.
  • Must be knowledgeable of reimbursement regulations and nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long-term care administration.
  • Must be familiar with the laws, regulations, and guidelines governing personnel administration.
  • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents, staff, and visitors based on whatever maturity level at which they are currently functioning.
  • Must be able to communicate policies, procedures, regulations, reports, etc., to personnel, residents, family members, visitors, and government agencies/personnel.
  • Ensure timely and accurate completion of all MDS social services sections in compliance with regulations.
  • Lead and coordinate care plan meetings, including initial, quarterly, annual, and discharge care plan conferences, ensuring required disciplines and family/resident participation.
  • Experience completing psychosocial evaluations, social histories, cognitive, mood and behavioral assessments.
  • Actively manage and coordinate discharge planning, including collaboration with IDT members, hospitals, families, and community resources.
  • Proactively address and resolve resident and family concerns, grievances, and psychosocial needs.
  • Provide psychosocial assessments, counseling, and support services as indicated.
  • Familiarity of Resident Rights and Advance Directives.
  • Maintain compliant, thorough documentation in the medical record
  • Collaborate closely with nursing, therapy, admissions, and administration to facilitate smooth transitions of care.
  • Adapt quickly to changing resident needs in a high-volume, short-term rehabilitation environment.
  • Will act as liaison and referral with psychiatric and psychological service providers as well as ancillary service providers.
  • Must be a team player with strong clinical skills.

Experience and Skills:
  • Master’s Degree, MSW required.
  • Must possess a working knowledge of long-term care operational standards set forth in the Federal Register, Requirements of participation. Experience working with the elderly.
  • Prior experience as a discharge planner in a Skilled Nursing Facility strongly preferred.
  • Experience in a short-term rehab–focused, fast-paced environment.
  • Strong organizational, communication, and conflict-resolution skills.
  • Ability to prioritize tasks and meet regulatory timelines in a dynamic setting.
