Position Description

The Social Worker position performs professional and technical duties involved in assisting individuals, families and/or groups of people by providing them with services designed to help them meet a variety of human, personal and environmental needs. Performs related duties as required.

Metropolitan Social Services (MSS), a department of Metropolitan Government of Nashville and Davidson County, focuses on topics and issues continually effecting the economic and social wellbeing of Nashvillians. National and local policy choices have resulted in a deterioration of standard of living and put more people into poverty. Poverty can be multidimensional and result from long-term processes of inequality, persistent exclusion, concentration of economic power, access to wealth-building and social capital, as well as broad transformations in work and society.

Typical Duties

  • Perform concise, comprehensive social assessments and developing service plans based on the customers individual needs, assessments of individuals which may include individuals with acute or persistent mental disorders or cooccurring disorders.
  • Provide information and referral services to customers and the public that enables them to utilize the programs services to best address their needs.
  • Provide short-term or long-term case management services to all populations served.
  • Conduct home visits to customers homes and/or other agencies/locations within the community and/or designated co-located partnership sites.
  • Research and advocate for customers to receive appropriate services through community resources such as public assistance programs, drug rehabilitation programs, and housing shelters.
  • Coordinate services, plan, and maintain ongoing contact with customers for completion of services.
  • Enter data into multiple case management systems and computer applications.
  • Submit monthly reports.
  • May be responsible for providing basic care, counseling, and assistance to individuals with mental illness or developmental disabilities.
  • Work with various established Metro partnerships as well as being flexible to work across the Division of Programs in any of the agency identified program units.
  • May perform case management and case review of assigned Conservatorship cases.
  • May provide written summaries and any reports from completed interviews from the assigned Conservatorship cases.
  • Build and maintain working relationships with various Metro Departments and other community agencies to ensure the emotional and social well-being of the assigned constituents are being met, assessed, and documented on a regular basis.
  • Attend community events.

Minimum Qualifications

EDUCATION AND EXPERIENCE:

Bachelor's Degree in Social Work from an accredited college or university

Valid Driver License
