Position Title: MSW Social Worker
Department: Social Services
Reports To: Director of Social Services
FLSA Status: Exempt - Learned Professional
Essential Duties & Responsibilities
The Social Worker exercises professional judgment to maximize residents' well-being and quality of life.
ADMINISTRATIVE
- Review facility policies and procedures with the IDT to assure compliance with state and federal regulations.
- Participate in development/maintenance of Social Services policies.
- Contribute to QAPI initiatives.
- Maintain updated community resource listings.
- Ensure documentation meets regulatory standards.
ADVOCACY
- Protect resident rights; act as mandated reporter.
- Support resident/family advocacy and empowerment.
- Identify system-level issues and recommend improvements.
CLINICAL
- Complete social histories and biopsychosocial assessments.
- Develop interdisciplinary care plans.
- Provide counseling, crisis intervention, grief support.
- Support residents/families during catastrophic events.
- Coordinate discharge planning and referrals.
EDUCATION
- Educate staff on psychosocial needs, aging, disability, cultural humility.
- Educate residents/families on rights, responsibilities, and problem-solving.
- Supervise social work students.
CONSULTATION
- Provide case consultation.
- Advise management on program planning and policy development.
Qualifications
- Master's Degree in Social Work (MSW) required; NYS LMSW/LCSW preferred.
- 5-10 year's experience in long-term care or healthcare social work.
- Proficiency in Microsoft 365 and EMR systems.
Physical Requirements
- Sensory skills: see, read, talk, hear.
- Manual dexterity for office equipment.
- Occasional bending, carrying, kneeling, reaching, pushing, pulling.
- Lift up to 20 lbs.
- Sitting/standing for extended periods.
- Reasonable accommodations provided when necessary.
Non-Physical Requirements
- Analyze psychosocial and clinical information.
- Prioritize caseload and formulate independent care plans.
- Provide crisis intervention and conflict resolution.
- Communicate with residents (including cognitively impaired), families, and staff.
- Maintain HIPAA confidentiality.
- Apply CMS/NYS DOH care planning and documentation standards.
