We are looking for a dedicated Social Worker to join our Adult Social Care team, working with adults under 75 who have a wide range of care and support needs. You'll undertake meaningful, strengths‑based work that enables people to achieve positive outcomes, stay safe, and maintain their wellbeing.

About the Role

As a Social Worker in our Health and Adult Service's, you will:

• Undertake asset‑ and strength‑based assessments with adults and carers.
• Determine eligibility under the Care Act and clearly communicate outcomes.
• Work creatively with individuals to develop personalised care and support plans that represent best value.
• Provide professional information, advice and guidance, exploring universal services, voluntary sector options, and alternative funding streams.
• Support the uptake of direct payments and initiatives such as reablement, personalisation, and self‑directed support.
• Undertake reviews, reassessments, and emergency interventions as required.
• Lead safeguarding adults' information‑gathering and investigations (following training and experience).
• Maintain high‑quality, timely case records in line with council standards.
• Work collaboratively with partners across health, community and voluntary sectors.
• Participate in the duty system and support performance data collection.
• This role will include opportunities to work alongside specialist teams, including Mental Health, Preparing for Adulthood, CHC, DoLS/Review, and Prevention, supporting your professional growth.
