We know - this is going to take some time to find just the right person - but it might be you!The position will require someone who can work independently. You will be responsible for managing a one person office - take phones calls, scheduling outings - and also being the person doing the outings while we grow! The position requires care management as well - someone who is experienced at coordinating client care. Hence the need for a social work degreeAs the office grows, your duties will change to more care managment, less outings and office management but you will be in on the ground floor (not literally, our office is on the second floor)If this sounds of interest to you, please reach out ASAP with your resume.Duties:Office duites:*Manage phone calls, outing requests*Schedule outings with company vehicles*As we grow, supervise new employees, schedule their outingsOutings:*Using company vehicles, escort clients to medical appointments and social/recreational outings*Complete notes about the outing and send to responsible partiesCare Management duties:*Conduct assessments, create and implement care plans*Attend home visits, medical appointments, care conferences*Liaiason between faclities, responsible parties, medical providers, and clients*Coordinate care, bring in resources, assist with movesQualifications:*Bachelor's or Master's in Social Work degree REQUIRED*Experience working with seniors and adults with disabilities*Excellent driving record and own car*CPR certified or able to obtain*Good observational and writing skills*Must have excellent listening skills with critical thinking*Computer skills requiredJob Types: Full time - 35+ hours a weekPay: $25 to $50 per hour DOE on role and experienceBenefits:Paid time offHealth insuranceSupplement health plansWork Location: In person (Portland metro) Equal Employment Opportunity StatementSeaStar Senior Services is an Equal Opportunity Employer.
