    Job Description

    Responsible for providing ongoing psychosocial assessment, treatment and discharge planning to a variety of patients within the medical setting. Provides clear communication across the continuum to improve patient outcomes. Responsible for an assigned caseload of patients.

    Work requires the knowledge of theories, principles, and concepts normally acquired through completion of a Master's Degree in Social Work field. Licensure as a Social Worker by the State of New Jersey required.
