Social Worker, Philadelphia County

Social Worker needed for temporary position working 20 hr/week in Philadelphia County.

We are seeking a Social Worker to provide social services support in a long-term care setting. In this role, you will complete psychosocial assessments, develop individualized care plans, coordinate with interdisciplinary teams, and connect residents and families with community resources. You will assist with discharge planning, Medicare and Medicaid applications, financial assistance, transportation needs, and insurance or billing concerns while maintaining accurate documentation and regulatory compliance.

The Social Worker will advocate for resident rights, educate residents and families on advance directives, address grievances, participate in care plan and quality improvement meetings, and ensure residents' emotional and psychosocial needs are met.
