Job Description:
- Evaluates the psychosocial needs of patients and families, developing tailored care plans that address psychological, social, functional, and financial impacts of illness.
- Travels to patient residences for initial assessments, identifying and assessing psychosocial needs and understanding caregiving dynamics.
- Assists patients and families in obtaining financial resources and provides education and assistance in completing Advance Directives.
- Offers individual or group counseling to patients and families, creating and implementing individualized discharge plans according to standards of practice and patient needs.
- Communicates effectively with caregivers, making referrals for intra-organizational services and community resources such as Medicaid, Salvation Army, and food banks.
- Serves as the liaison to payer sources, obtaining authorization and updating as needed. Interacts with facility/hospital case managers to ensure the provision of appropriate hospice services.
- Evaluates family care unit bereavement needs throughout the hospice process, completing referrals to the Bereavement Department and documenting appropriately.
- Maintains appropriate professional documentation in the electronic medical record, ensuring accurate and timely completion.
- Participates as an active member of the interdisciplinary team, attending all scheduled meetings.
- Assesses the need for volunteer assistance, provides non-medical on-call services as scheduled, and participates in staff meetings, department meetings, in-services, and other related activities.
- Attends educational and developmental seminars and meetings as deemed appropriate, practicing principles of employee and patient safety, and complying with all local, state, and federal regulations.
- Performs other duties as assigned.
Knowledge, Skills, and Abilities:
- Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]
- Knowledge of electronic medical record. [Required]
- Ability to follow standard precautions and uses personal protective equipment when indicated. [Required]
- Ability to complete hospice Orientation and Training, as necessary. [Required]
Education:
- Bachelor's [Required]
- Master's (MSW [Required]
Field of Study:
- In psychology, sociology or social work
Work Experience:
- 2+ experience in the social work field [Required]
- Healthcare/hospice experience [Preferred]
