The duties of this position include but are not limited to the following:

The GS-13 Enhanced Team Coordinator is an advanced practice Social Worker assigned to the Housing and Urban Development/VA Supportive Housing (HUD-VASH) program within Community Reintegration Service (CRS). Organizationally, CRS is under the Mental Health Division and the Enhanced Team Coordinator is directly supervised by the CRS Director. The incumbent of this position has responsibility for coordinating interdisciplinary staffing, work assignments, budget, treatment services provided, and admission criteria for the programs. Incumbent supervises the programs, and this includes orientation and training and assigning and monitoring clinical care. Incumbent is responsible for reviewing work for performance and completing all required performance reviews. Performance plans for incumbent are in Executive Career Field (ECF).

CRS is comprised of a multidisciplinary team responsible for providing low barrier services to chronically homeless Veterans or Veterans with significant barriers to housing and employment stability due to severe mental illness, substance use disorders, chronic disabling health conditions and sometimes criminal backgrounds. The Enhanced Team Coordinator is responsible for direct supervision of multidisciplinary clinical team that supports HUD-VASH Social Work teams. The multidisciplinary team consists of Psychiatric Nurse Practitioner(s), Psychologist, Registered Nurse(s), Occupational

Therapist(s), Recreational Therapist, Substance Use Disorder Specialist(s), Peer Support Specialists, and Housing Specialists. In addition to providing direct supervision to HUD-VASH multidisciplinary team, the Enhanced Team Coordinator is responsible for developing and maintaining employee orientation and training processes for multidisciplinary team and functioning as compliance expert for larger program. The incumbent may act as the Contracting Officers Representative (COR) or oversee the work of the COR for the program, the includes the Emergency Transition Housing programs, in addition to oversee associated budget. The incumbent of this position manages mental health access and triage processes across all CRS programs.

The Enhanced Team Coordinator is responsible for coordinating multidisciplinary team access across multiple CRS programs and this includes telehealth access to patients served in rural communities. The Enhanced Team Coordinator is responsible for ensuring that assigned teams provide care that is consistent with all Homeless Program Office Directives including VHA Directive 1162.0 Housing and Urban Development Department of Veterans Affairs Supportive Housing Program. The Enhanced Team Coordinator co-leads orientation and training of all CRS staff. Enhanced Team Coordinator trains the supervisory group on how to assign and monitor clinical care using clinical acuity model. Incumbent is responsible for reviewing work for performance and completing all required performance reviews.

Community Reintegration Service at VAPORHCS serves very large catchment area. Supervisory responsibility for HUD-VASH is divided by geography that includes North Team, South Team, Rural South Team, Central Oregon Team, and Multnomah County East Team and Multnomah County West Team. The Enhanced Team is responsible for working with the entire HUD-VASH team across multiple communities. The entire Supportive Housing program is responsible for providing case management and oversight for more than 2400 Veterans who are utilizing HUD-VASH vouchers and supportive housing services through 14 different Public Housing Authorities.

Qualifications

Basic Requirements:

  • United States Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens).
  • Education: Have a masters degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the masters degree in social work.
  • Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the masters degree level.
  • English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed.

GS-13 Social Worker (Program Coordinator) Grade Determination:

  • Experience and/or Education: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-masters degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
  • Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
  • Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
  • Skill in coordinating and implementing one or more specialty program(s), across the facility.
  • Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
  • Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  • Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
  • Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
  • Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.
  • Knowledge of organizational structure and impact on program operations.

Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
