Position Overview:

Our client is seeking a dedicated and compassionate Social Worker to join their team, playing a key role in providing high-quality, comprehensive care to frail seniors. The Social Worker will work closely with a diverse team of healthcare professionals to ensure that patients receive the best possible support and services. This role is ideal for individuals who are passionate about making a positive impact in the lives of seniors and fostering a holistic approach to their care.

Setting: Outpatient Clinic working with the geriatric population.

Responsibilities:

  • Conduct social work assessments to identify the psychosocial needs, preferences, and goals of patients, contributing to the development of personalized care plans.

  • Actively participate in team meetings to collaborate on care plan creation and patient management.

  • Provide social work interventions as outlined in patient care plans, including resource arrangement, care transitions, individual and group counseling, and case management.

  • Partner with primary care physicians and other team members to ensure smooth transitions of care between settings, such as hospitals, skilled nursing facilities, and home care.

  • Coordinate, facilitate, and lead care conference meetings to ensure effective communication and care coordination with patients, families, caregivers, and team members.

  • Offer discharge planning and support when patients disenroll from the program.

  • Other duties as assigned

  • Master’s Degree in Social Work (MSW).

  • A minimum of one (1) year of experience working with a frail or elderly population.

  • Experience coordinating and facilitating care conference meetings.

  • Perfered experience working with individuals who have behavioral health and substance abuse issues.
