Description + Assists patients with navigating the Department of Children and Families (DCF) process + Coordinate services to home care and equipment following discharge + Documents pertinent information to facilitate care coordination + Participates in care conference activities + Refers to rehab and long term care facilities + Refers to other disciplines/resources + Participate in development of treatment plan Counseling/Therapy: + Individual counseling + Marriage counselling + Parental counseling Skills + epic, LCSW, LICSW, Social work, Mental health, Behavioral health, LMFT, LMHC, care coordiantion, risk assesment, counseling, therapy Additional Skills & Qualifications + Licensed as a Mental Health Counselor, Marriage and Family Therapist, or Independent Clinical Social worker Experience in conducting crisis risk assessments for adults and pediatrics Hospital experience preferred + Hospital experience preferred + BLS through AHA required Knowledge of Community programs (income/insurance): + AFDC (Aid to Families with Dependent Children) + Food stamps + Medicaid + Medicare + Medication Assistance + TANF (Temporary Aid for Needy Families) Knowledge of Protective services: + Adult protective services + Child protective services + Emergency detention + Guardianship + Power of Attorney Knowledge of specialized areas: + Adoption / Foster Care + Drug/alcohol use and treatment options + Psychiatric Hospital + 5150/ Involuntary holds Job Type & Location This is a Contract position based out of Spokane, WA. Pay and Benefits The pay range for this position is $40.00 - $50.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
