Description: This is a combined posting for an Social Worker LCSW, Professional counselor, Licensed Professional Counselor, Mental Health Therapist LMFT and Social Worker MSW. + The Social Worker, LICSW will function as a core member of a multidisciplinary team and communicate patient's health care needs with primary care, behavioral health services, health care facilities and/or payers. Acts as an advocate and helps patient gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse. + The Professional Counselor (Mental Health Counselor Associate) practices in a treatment setting under clinical supervision and is dedicated to evaluation of psychiatric and substance use disorders. The Professional Counselor (Mental Health Counselor Associate) will collaborate with patient/support system to evaluate patients. + The Licensed Professional Counselor (Mental Health Counselor) practices in a treatment setting under clinical supervision and is dedicated to evaluation of psychiatric and substance use disorders. The Licensed Professional Counselor (Mental Health Counselor) will collaborate with patient/support system to evaluate patients. + The Mental Health Therapist LMFT practices in a treatment setting under clinical supervision and is dedicated to evaluation of psychiatric and substance use disorders. The Mental Health Therapist LMFT will collaborate with patient/support system to evaluate patients. Core Responsibilities: Provides comprehensive biopsychosocial/spiritual assessment, interventions/advocacy of patients and their support systems. The Social Worker incorporates concepts and theories associated with life span development, neurobiology, and behavioral change. The Social Worker will collaborate with the patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter-Multi-Disciplinary Team (IDT). + Assists patients with navigating the Department of Children and Families (DCF) process + Coordinate services to home care and equipment following discharge + Documents pertinent information to facilitate care coordination + Participates in care conference activities + Refers to rehab and long term care facilities + Refers to other disciplines/resources + Participate in development of treatment plan + Provide counseling/therapy (individual, marriage, parental) Additional Skills & Qualifications: + Washington Licensed Mental Health Counselor Associate, Marriage and Family Therapist, Clinical Independent Social Worker, Clinical Independent Social Worker Temporary, Mental Health Counselor, Advanced Social Worker, Advanced Social Worker Associate, Clinical Independent Social Worker Associate + Experience in conducting crisis risk assessments for adults and pediatrics Hospital experience preferred + 2 years of ambulatory clinic or hospital experience preferred + BLS through AHA required + EPIC Knowledge of Community Programs (income/insurance): + AFDC (Aid to Families with Dependent Children) + Food stamps + Medicaid + Medicare + Medication Assistance + TANF (Temporary Aid for Needy Families) Knowledge of Protective Services: + Adult protective services + Child protective services + Emergency detention + Guardianship + Power of Attorney Knowledge of Specialized Areas: + Adoption / Foster Care + Drug/alcohol use and treatment options + Psychiatric Hospital + 5150/ Involuntary holds Job Type & Location This is a Contract position based out of Spokane, WA. Pay and Benefits The pay range for this position is $40.00 - $50.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
