Provides psychosocial assessments, assists in care planning in coordination with the members of the Palliative Care team, Hospital Staff and Physicians. Provides counseling services to patients/families. Demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.
MINIMUM REQUIREMENTS
Education, Credentials, Licenses:
MSW from accredited school by the Council on Social Work Education. Licensed Clinical Social Worker or Certified Social Worker willing to participate in active supervision toward achieving LCSW.
Specialized Knowledge:
Knowledge of Palliative Care and the psychosocial issues of terminally ill patients and their families. Excellent communication skills and the ability to coordinate care well with an interdisciplinary team. Knowledge of Emotional and Mental Disorders and the DSM V diagnoses applicable to palliative care.
Kind and Length of Experience:
2 years of social work experience in a medical setting.
