Responsibilities:

We are looking for a Social Worker to join our team within our youth Partial Hospitalization Program.  As a Social Worker, you will collaborate closely with psychiatric medical providers and therapists on the patient’s primary treatment team to ensure a cohesive and effective plan of care.

Hours for this role are Monday – Friday, 8a-430p

Responsibilities and Accountabilities:

  • Assessment and Planning: Conduct effective assessments and suicide risk evaluations, develop treatment and discharge plans, and lead educational or therapeutic groups.
  • Patient and Family Communication: Maintain open lines of communication with patients and their families to ensure they are actively involved in treatment decisions.
  • Care Coordination: Coordinate care with outpatient providers during patient admissions and discharges to ensure continuity of care.
  • Team Facilitation: Facilitate weekly treatment team meetings, creating and updating master treatment plans based on team consensus.
  • Clinical Contribution: Bring a unique clinical perspective to the interdisciplinary team, drawing on training in family systems and social work theory
Qualifications:
Education:
Bachelors Degree in Social Work required, Master’s Degree in Social Work highly preferred, with licensure in the State of Minnesota required.
 
 
Previous Experience:
    • Experience in mental health highly preferred
    • Experience using electronic health records preferred
    • Proficient in Microsoft Office Products including Word and Outlook
    • Proficient in Microsoft Office Products including Word, Outlook, and Teams.
    • Able to effectively and efficiently communicate in written and verbal form.
    • Ability to smoothly and rapidly shift between dissimilar tasks.
    • Ability to respond appropriately in times of mental health crisis.

Compensation for this role ranges from $27-$35 per hour depending on licensure and experience.
