Duration:  2years Contract
Location: Stormville, NY 12582
Shift Timings: Monday –Friday
Pay- USD 35/hr-USD40/hr

Position summary:
This position is a part of the Office of Mental Health(OMH). OMH provides mental health services to forensic patients. Candidate will be required to submit of TB testing and recent flu shot. May have contact with incarcerated individuals.
 
Key Responsibilities:

• Contract staff would mainly be working weekdays and assisting in providing therapeutic services to patients in a primary therapist role to include individual therapy.
• Responsible in completion of mental health assessments and documentation (Core History, Suicide Assessments/risk assessments, Weekly/Monthly Progress Notes, Treatment Plans, Discharge Plans), according to policy and procedure.
• Group facilitation will also be part of the expectations.

 
Certification:

• Licensed Master Social Worker or Licensed Clinical Social Worker
