The Chippewa Falls Area Unified School District is accepting applications for a Special Education Transition Coordinator for the 2026-27 school year that will be at Chippewa Falls Senior High School, but will travel to muliple sites.
Primary Purpose:
The Post-Secondary Transition Coordinator guides students with disabilities and their families through the transition process from the K-12 system to individualized post-secondary life. The Transition Coordinator presents options related to postsecondary education, employment, and long-term support, and assists students and families in connecting with relevant agencies to support them in this process.
Essential Duties & Responsibilities:
- Regular and Reliable Attendance
- Coordinate programming at the Cardinal Transition Academy, including supervising educational assistants and a vocational coordinator from Aurora Vocational Services
- Coordinate all aspects of the Goodwill Project course, including curriculum planning, design, and preparation, providing daily instruction, and scheduling
- Case manage students at the Cardinal Transition Academy and Project SEARCH sites
- Serve as a resource to teachers in the development of transition activities, curriculum, and assessment
- Coordinate transition activities for college, career, and community readiness
- Develop and provide resources to teachers, students and families about the process of transition to adulthood
- Facilitates referral, application, eligibility process for students and families to adult service providers
- Assist parents/students in coordinating meetings with outside agencies
- Review and submit Postsecondary Transition Plans within the DPI portal
- Coordinate transportation to transition related programs
